Case Planning as Part of the SDS Mentor Program
As part of the SDS Mentor Program, you have the opportunity to plan specific patient cases together with your mentor. To do so, please provide your mentor with all relevant information related to your case, including the patient’s medical history, X-rays, photographs, CBCT data, and any additional diagnostic records.
Please compile all information and files you would like to submit into a .zip file and upload the file using the form.
Please describe your case in as much detail as possible. Based on the information you provide, your mentor will develop an individualized treatment proposal and provide specific recommendations for the next steps.
As part of the SDS Mentor Program, SDS covers the cost of up to five individual Case Planning consultations. To redeem this benefit, simply enter your personal voucher code when booking. The voucher can be used for a total of up to five Case Planning consultations.
