BONE AUGMENTATION WEEK

Become a specialist in small and complex bone reconstructions within one week. The Bone Augmentation Week is divided into 3 levels:

LEVEL 1 – FOCUS: SOFT AND HARD TISSUE

Perhaps you, like us, once learned about bone healing mechanisms and things like osteo-progenitor cells at university, but today you might not be able to recall them perfectly. And in most cases, it works fine without this knowledge: most extraction sockets heal, and bone formation occurs quite well when we apply the products from Company X or Y as intended...

But as always, we and the participants who come to us want more! We want to pull out all the stops to regenerate or build bone absolutely safely, reproducibly, and predictably. And this increasingly without limits, even in challenging situations!

Therefore, our Bone Augmentation Week starts with a focus on bone, delving into bone metabolism on a molecular level from Vitamin D3 to K2, Osteocalcin, and macroscopic mechanisms such as cavity formation, Healing Chambers, and the influence of bone preparation, as well as the osteoinductivity of the periosteum and the Schneiderian membrane. Furthermore, we examine bone and defect healing mechanisms and the criteria for successful bone augmentation, with special consideration of various bone substitute materials and membranes. Only when you master these mechanisms thoroughly can you decide which type of bone substitute material is appropriate for each situation: fast or slow resorbing, do we need some autogenous bone or blood concentrate to biologize the augment, which membrane makes sense, and what are the precise mechanisms when these are digested step by step? We will open the door wide for you, organizing and indexing these materials synthetically, vegan, bovine, porcine, equine, allogeneic, and autologous. After this first day, you will be absolutely confident discussing bone healing, bone substitute materials, and membranes with any expert and be able to independently choose the right material/product for any indication!

We will also dive into the topic of soft tissue and wound healing. This naturally starts with incision design! The goal of intelligent incision design should be to minimize vessel and structure damage, scar formation, and maintain optimal blood supply with atraumatic, minimally invasive techniques, avoiding postoperative morbidity for your patients! You will revisit the mechanisms of soft tissue and wound healing, recalling the three phases "inflammation, proliferation, and tissue remodeling".

New to us was the influence of blood concentrates on these three phases and how they intervene to optimize and accelerate this biological healing process, ensuring that the three phases occur almost simultaneously and generate security through acceleration. The Low-Speed Centrifugation Concept (LSCC) is a product- and company-independent centrifugation protocol developed by Prof. Dr. Shahram Ghanaati, which produces various matrices as a solid form (formerly A-PRF) and as an injectable or wettable liquid form (formerly I-PRF). You will get an overview of which speeds you need to use for other centrifuges, e.g., those from BTI or Curasan, to reach a specific G-Force as provided in the LSCC. You will understand how, depending on the indication, you can obtain firmer, thicker, or thinner matrices or the liquid form, which improves soft tissue, or produce Sticky Bone. In this level, we will also expand on the intelligent use of collagen membranes, which play an essential role in wound healing and must be biologized with blood concentrates. You will agree that the postoperative flap's blood supply has a tremendous influence on wound healing, complication rate, and dehiscence tendency. Therefore, we will extensively cover the flap's support with liquid injections and intelligent suturing techniques, such as the Apical Mattress Suture, which prevents tension on the flap or strangulation of blood supply. You will be thrilled when the OPEN HEALING PROTOCOL by Prof. Dr. Shahram Ghanaati is presented, which is the only concept capable of covering extensive augmentation volume increases in the coronal direction without flattening the vestibulum and without provoking completely mobile gingiva.

We have operated many of these OPEN HEALING situations in recent years at the SWISS BIOHEALTH CLINIC and are fascinated by how well, predictably, and elegantly extensive augmentations, such as those in the context of BISS-OP, can be covered and how pain and swelling can be avoided since no reliefs or periosteal slits need to be performed. However, it is essential that you later adhere extremely diligently to the proven protocol, which we will present on this second day, for your own cases.

LEVEL 2 – LIVE OPS

You might be thinking: "All well and good, but let's see it in action! I would like to see how this works on real patients!" That's precisely the strength of the SWISS BIOHEALTH EDUCATION CENTER: everything you learn here has been developed and tested within a radius of 200 meters and proven a thousand times over. So, now it's our turn, and on this day of the course, you can sit back and watch us operate with a gin and tonic or a glass of champagne in hand. Together with Prof. Shahram Ghanaati, the team from the SWISS BIOHEALTH CLINIC, including Dr. Karl Ulrich
